Refer a caregiver

Référence aidance Québec

Best practices recommend the rapid referral of caregivers and their loved ones to services that meet their needs. The Government of Quebec, L’Appui pour les proches aidants and the Federation of Quebec Alzheimer Societies have joined forces to develop an online referral tool for health care professionals.

Why refer a caregiver?

referenceaidancequebec.ca is an easy-to-use tool that facilitates the referral of caregivers and allows them to access the support they may need more quickly, no matter what their situation.

Référence aidance Québec is in line with the Act to recognize and support caregivers and the National Policy (in French only). It stems from the collaboration and referral agreement between the Government of Quebec, L’Appui pour les proches aidants and the Federation of Quebec Alzheimer Societies, which came into effect during phases 1 and 2 of the Québec Alzheimer Plan.

What is the referral process?

With the caregiver’s consent, the member of the health network (doctor, social worker, nurse, etc.) fills out a short form on referenceaidancequebec.ca to refer the caregiver to experts who will listen to the caregiver and steer them towards appropriate solutions. The person referred is contacted within 10 business days by counsellors from the Caregiver Support Helpline or the Alzheimer Society in order to:

  • identify their needs;
  • share information;
  • introduce them to services and direct them to local resources;
  • ensure a punctual follow-up, according to their needs, if they so wish.
